A setback was delivered last week to doctors who want to extend the use of stents beyond the treatment of coronary artery disease. A favorite therapeutic target for proponents of stents has been the treatment of blockages in the carotid artery - the large artery in the neck that supplies blood to the brain. Blockages in the carotid artery (called carotid artery stenosis) can lead to strokes and transient ischemic attacks (TIAs - so-called "mini" strokes, in which the neurological deficit resolves within 24 hours). Traditionally, vascular surgeons treat carotid stenosis by a surgical procedure called carotid endarterectomy - where they surgically "ream out" the blocked artery. In recent years, however, other physicians have advocated the much less invasive approach of stenting the blocked carotid artery, the same way blocked coronary arteries have been stented. Stenting, it has been assumed, should be safer than the traditional approach. But last week at the European Stroke Conference in Stockholm, investigators from London reported on safety results from the International Carotid Stenting Study. In this study, over 1700 patients with carotid stenosis were randomized to either surgery or stenting. While the long-term results from the trial will not be available until 2011, the 30-day post-procedure results (reflecting the relative safety of the two procedures) were reported last week. These results showed a significantly higher incidence of early stroke (within 30 days of the procedure) with stenting as opposed to surgery. In fact, strokes occurred after stenting nearly twice as often as after surgery. These results are consistent with results from two earlier trials comparing surgery with stenting for carotid stenosis. Neither one showed a more favorable safety profile with stenting, and one of them, like the current study, suggested stenting was less safe. While it is still possible that the long-term results may show that stenting ultimately is superior to surgery, even that result would require a patient to accept increased early risk for a reduced long-term risk. In any case, for now, surgical carotid endarterectomy, and not stenting, should remain the standard surgical approach to treating carotid stenosis.

Specifically, the program emphasizes walking at a moderate pace for at least 45 - 60 minutes per day, at least 5 - 6 days per week. For most individuals, this program will result in burning an "extra" 3000 - 3500 calories per week, which translates to nearly 1 pound of weight per week. (Typical rehab programs of shorter, higher-intensity exercise result in about 800 calories burned per week.) This new kind of rehab program also emphasizes counting (and limiting) the number of calories eaten, so that the overall result is designed to be a sustainable loss of weight. In a small clinical trial involving 74 patients with significant coronary artery disease, those receiving the "calorie-burning" variety of cardiac rehab lost more weight, more total fat, and more abdominal fat (and more waist circumference) than those receiving the standard form of cardiac rehab, and had a significantly greater improvement in their cardiovascular risk profile (including cholesterol values). The significant improvement in cardiac risk persisted for the 1-year follow-up evaluation. This small clinical trial will certainly not be sufficient to change clinical practice or formal medical guidelines. But it does point out that a) there is more than one way to accomplish cardiac rehabilitation, and b) a calorie-burning walking program can significantly improve your cardiac risk, whether you have known coronary artery disease or not. A 27 year old Asian male comes to see you because he has not been feeling well. He has lost weight and says he has no appetite has vague chest pain, a persistent cough, is unable to sleep lying down and is not able to work. For the past 2 years, his abdomen has been getting distended and his physician put him on a water pill. He did not have money to get any type of medical care and now is admitted with dyspnea. He has no other past medical history. He denies smoking, use of drug or any other health problem. He was born in Thailand in the country side. Physical reveals an emaciated male, with a BP of 100/65, P irregular and not palpable, T 37, RR 22. Auscultation reveals that he has a very loud S1. Your ability to decipher murmurs suck and you are not able to tell whether there is a systolic or diastolic murmur- but you do hear a noise. You also hear an extra heart sound which sounds like a thump and you are sure it occurs after the 2nd heart sound. Blood work is unremarkable except for a mild anemia. The patient does get an ECG and a Chest X ray. The only thing you can tell from the ECG is that the heart rate is irregular and you cant even find a p wave. Your medical school sure did not teach you how to read an ECG. The chest x ray shows an enlarged cardiac silhouette and mild pulmonary congestion. Based on the history and physical, answer the following questions?

While isolated systolic hypertension(ISH) has been identified as a specific clinical entity, isolated diastolic hypertension(IDH) has not been reported as a separate group. When we analysed our data from our hypertension clinic we found a distinct subgroup of patients who had elevated diastolic blood pressure with normal systolic pressure. We report the clinical profile of these patients. 440 newly registered hypertensive patients between the year 1998-99 formed the study population. All patients with secondary hypertension were excluded.. IDH was defined as diastolic BP more than 90mmhg and systolic BP less than 140mmhg.
IDH was present in 32(7.2%) patients. The male female ratio was 3:1, mean age was 42(Range32-56) The mean diastolic pressure was 96 mm (Range 90-110).The mean systolic pressure was 136mm(Range 128-140). LVH was observed in 4 patients(12.5%). Diastolic dysfunction was detected by echocardiography in 20patients.(62%)
We conclude that isolated diastolic hypertension constitute a significant subset among hypertensive patients and they need further study regarding the pathogenesis, clinical presentation and therapeutic implication.
